1. In those above age 60, Get the blood pressure below 150 sytolic and 90 diastolic.
2. In those below age 60, diabetics, and those with kidney disease, get the blood pressure below 140 sytolic and 90 diastolic.
3. In blacks including those with diabetes, Start with thiazide diuretic or calcium channel blocker (amlodipine).
4. In non-blacks including those with diabetes, Start with thiazide diuretic, calcium channel blocker, ACE inhibitor (Lisinopril), or ARB inhibitor (losartan).
5. In all over 18 with chronic kidney disease start with or add ACE inhibitor (Lisinopril), or ARB inhibitor (losartan).
6. Adjust /add medications monthly up to three drugs.
7. Do not take ACE inhibitor (Lisinopril), or ARB inhibitor (losartan) together.
8. Refer early to a hypertension specialist who is actually a Nephrologist and not a cardiologist if can not control the blood pressure.
